Some Phoenix-area businesses plan to require vaccine proof | 3 On Your Side

PHOENIX (3TV / CBS 5) – If you were vaccinated months ago, do you remember where you put your vaccination card? Some US cities require proof of vaccination for certain indoor activities, and more and more companies are demanding it.

In New York City, you will now need to show your vaccination certificate for certain indoor activities such as eating at a restaurant. Later this week, San Francisco will require proof of vaccination when entering bars, restaurants, or theaters.

More US cities that need proof of vaccination to visit places

We don’t have anything like this in the Phoenix area, but we do know of a few places that require proof of vaccination, like FnB in Old Town Scottsdale. From next week, the Merc Bar in the Biltmore area will also require proof of vaccination or a negative Covid test. As of mid-September, venues like the Crescent Ballroom, Marquee Theater, Valley Bar, Rebel Lounge, Celebrity Theater, and more will require proof of vaccination or a negative COVID test if you want to go to a concert or event. So what if you lost your vaccination card?

Dr. Rebecca Sunenshine of Maricopa County Public Health says you have two options. You can request a copy of your vaccination certificate from the Arizona Department of Health Services. Click here to be redirected to the website. However, according to Sunenshine, the fastest way is to sign up for MyIR, where you can get a copy of your and your family’s vaccine information.

“No matter where you got your vaccination, as long as it has been reported to the Arizona Department of Health, which was required by law to be entered into this system, you can get the data from the system through any of these processes,” Sunenshine said.

Even if you have your vaccination card, Dr. Sunenshine, make a copy of this, laminate it, and keep that copy with you. You’ll also want to keep your original in a safe place in case you someday need a booster shot and need to update it. If you haven’t already, take a photo of your card as well.
